Student Consent Form: Mulitmedia for Public Viewing
- This form is to be completed by students who need financial assistance with purchasing school supplies. To qualify, you need to be an Indigenous student who lives on Seabird Island and attending a school off reserve (Seabird Island Community School students are provided school supplies so are excluded). Please send completed forms to: eatoedu@seabirdisland.ca
- Should you wish to have your funds deposited directly into your account instead of cheque by mail or pick up, please complete this form and send to Seabird Island Band accounts payable.
Parent/Guardian Release of Student Information Form
- Seabird Island Band has a Local Education Agreement with School District 78, a parent waiver is no longer required. All students attending private schools, or other districts such as Chilliwack SD33, must complete this waiver in order to provide permission to the school to confirm student attendance, grades, etc.. This information is needed to provide student support and provide education financial incentives and supplements. Please send completed forms to: eatoedu@seabirdisland.ca
